There are three sets of codes available in the CPT book

Combined Arterial with Vein Graft (33517 - 33523)
- These combined A&V Graft codes are used with Arterial graft codes only to indicate the number of vein grafts utilized during the same encounter.
Coding Guidelines: Vein-Graft
v Use these code if only vein graft is utilized.
v Procurement
of the saphenous vein graft is included in the CPT codes 33510 – 33516.
Additional Billable service along with Bypass.
ü Harvesting of an upper extremity vein – 35500
ü Harvesting
of the femoropopliteal vein – 35572
ü Percutaneous
VAD – 33990-33993

Combined
graft +add on code for the vein (33517 - 33523) is used only with the artery
graft codes (33533 - 33536) if artery and vein graft is used for the coronary bypass procedure.

The below +add on codes are used along with bypass procedure codes,
CPT + 33508 (Endoscopy, a video-assisted harvest of the vein(s) for coronary artery
bypass procedure).

CPT + 33530 (Reoperation, coronary bypass procedure or valve procedure more
than 1 month after an original operation)

CPT + 33572 (Coronary Endarterectomy, open, of LAD, circumflex, or right
coronary artery performed in conjunction with coronary artery bypass graft
procedure, each vessel).
