Ultrasound Documentation:

Complete Vs Limited

To code complete – the report should contain a description of elements or the reason an element could not be visualized (Surgically absent).

For example, 76700 - US Abdomen Complete study (Liver, Gall Bladder, CBD, Pancreas, Spleen, Kidneys, Aorta and IVC)

If anyone of the required an element for a complete exam is not documented, it would be coded as limited (76705).

For US bladder correct CPT is 76857 it’s not 76775.

Appropriate modifier 59(XE, XP, XS, XU) should be used while coding both complete and limited study performed based on medical necessity and the documents support for the service.
